I recently had to replace a server that had the APM agent running. I've installed the agent on the new server, but I have now lost all the history associated with the previous agent. How do you maintain this from server to server?

Hi,
Let me clarify few things. If you haven't deleted the instance from Site24x7 APM console, you can still see the old data of your agent, even though the old server doesn't exist.
The new agent in the new server will be added as a separate instance (as host name is different). But, if you add it using the same application name in apminsight conf file, then the application view will have all the data put together. (both old and new server's data).

Hope this helps.
